Service Grid Revisions Effective 8/15/08

Service Grid Revision:
The Service Grid is being revised effective 8/15/08.  The new Service Grid version date is 8/15/08.  These revisions are taking place consistent with changes in DHHS- Office of Child and Family Services requirements.

1. This change effects only these procedure codes:
1. H2033 (Child & Family Behavioral Health Treatment Services- MST or “65M-MST”) & H2033HK (Child & Family Behavioral Health Treatment Services- MST, Problem Sexualized Behaviors or “65M-MST-PSB”)
2. The initial authorization for these 2 codes for the PA is now 150 days and 352 units. 
3. G9007HT (65M Collateral-MST) & G9007HK (65M Collateral-MST-PSB)
4. The initial authorization for these 2 codes for the PA is now 150 days and 100 units.

Any Continued Stay Requests for these services beyond 150 days are regarded as an exception, and will be reviewed by the APS Medical Director.
