APS CareConnection® Changes Effective 8/1/09

Consistent with the DHHS Co-Occurring Screening Tool Requirements, the following fields will be added to APS CareConnection® effective 8/1/09.  All providers will be required to complete these fields starting 1/1/10.

On the Treatment and Service History page, the following text will be added:

“Select the tool or tools used to screen for co-occurring mental health & substance use disorders.  Next to each selected tool, indicate if there were one or more YES responses.  If the AC-OK is used, answer for all 3 domains.  If the MHSF III is used, CRAFFT or UNCOPE must also be used”

The following options and corresponding check boxes and dropdowns will appear
beneath this text:

For PASRR & LTSE Services, the following fields will be added into CareConnection® specific to PASRR and LTSE Services, effective 8-1-09:

  • On the Member Information page: a field for “Medicare #” will be added next to the “MaineCare ID” number field.

  • On the Psychiatric Medications page, a text field will be added beneath the “Add Medication” button, and will be labeled: “Additional Medication Info (Non-prescribed, vitamins, supplements, dosage, frequency, etc.”

  • On the Additional Reporting Data page, a new section entitled “PASRR Level II” will be added. Within that section, the following series of boxes will be added:

    Current Physician(s)
    Duration of Diagnosis
    Medical History
    Medical Devices
    Neurological Systems
    Number of visits in the past year
    Functional Evaluation

    Each of these sections will contain corresponding questions and drop-downs.
