Simplified Outpatient/Med Management Review Effective 8/1/08

A simplified Continued Stay Review for Outpatient and Medication Management Services is in effect as of 8/1/08.  This simplified review applies to the following services and procedure codes:

• Services: Outpatient and Medication Management- adult, child, substance abuse, and psychological services for assessment, individual/family and group
• Procedure Codes: H2000; H2000HH; H0004; H0004HH; H0004HQ; H0004HQHH; H2010

APS CareConnection® Instructions for the Simplified Outpatient/Med Mgmt CSR

The Simplified Continued Stay Review (CSR) starts in the same manner that you have used previously to create a Continued Stay Review.  There is nothing new to learn, just less to fill out. APS CareConnectionÒ automatically fills in much of the information for you.  Start per usual at Search Request, clicking EXT from the Case ID you wish to extend. This will open up a Review, where you can click directly on the pages that you need to update, as instructed below.  To get to the page you need to update, click directly on its name in the left hand menu. Click Save and Continue after each page you update (this will always bring you to the next page).

Go directly to:

• Administrative – Enter the start date (date new units start).
  Click Save and Continue.  Go directly to:
• Multiaxial Assessment page - Enter Primary, and other diagnoses.
  Click Save and Continue, which will bring you to:
• Services Requested - Click Modify to update dates and units. 
  Click Save. Go directly to:
• Psychiatric Medications – Answer the questions and add medications (if known).
  Click Save and Continue, which will bring you to the:
• Clinical Indicators –Choose dropdown menu answers to the Risk/Danger to Self/ Others questions.
  Click Save and Continue, and then go directly to:
• Additional Info – Choose Treatment Progress from drop down menu.
  Click Save and Continue, which will bring you to:
• Submit to APS – Click Submit to APS, and you have finished your Simplified Continued Stay Review!

This simplified CSR may only be used for Outpatient and Medication Management Services.
